DAY SHIFT – PART-TIME – MONDAY THROUGH FRIDAY (Day Shift, 5 hour shift)

The Restorative & Functional Maintenance Nursing Assistant provides each of his/her assigned residents with restorative nursing care and functional maintenance program services in accordance with the resident’s assessment and care plan. May also be required to daily act in capacity of Rehab Tech to facilitate provision of appropriate levels of rehabilitative therapies.

Duties and Responsibilities
The following is a general list of key job duties and responsibilities: The list is not to be considered inclusive and you will be required to perform other duties/responsibilities as assigned by an authorized supervisor.
&#8226 Record all restorative and/or functional maintenance program documentation entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
&#8226 Completes daily, weekly and monthly documentation in accordance with restorative program and/or functional maintenance program guidelines with oversight by Clinical Supervisor (Rehab Director / Facility Restorative Nurse).
&#8226 Work closely with therapists / Facility Restorative Nurse to establish restorative nursing goals and interventions for the resident’s care plan.
&#8226 Follow established safety precautions in the performance of all duties.
&#8226 Under the direction of the Rehab Director / Facility Restorative Nurse, optimize abilities and skills of patients in facilities:
&#8226 Transport and move patients as directed.
&#8226 Assist licensed personnel in patient care.
&#8226 Assist with maintaining cleanliness of therapy equipment and area.
&#8226 Maintain awareness of change in patient population, programs, and patient conditions and inform licensed personnel.
&#8226 Establish and maintain strong relationships with professional staff and
good public relations with patients and families.
&#8226 Maintain confidentiality of company, patient and client information.
&#8226 Adhere to the appropriate dress code for assigned facility and work
&#8226 environment.
&#8226 Adhere to the requirements of the patient’s Bill of Rights.

Administrative Skills:
&#8226 Complete appropriate forms and records, as directed, make copies,
and obtain necessary signatures.
&#8226 Ensure that written and oral communications are directed to
appropriate personnel.
&#8226 Follow established protected health information (HIPPA) policies to
prevent privacy or security breaches. If breach identified, report to Broad River Rehab Management.

Other Physical Considerations
&#8226 Twisting, Bending, Crawling, Squatting, Kneeling, Crouching, Climbing and Balancing is required.

&#8226 Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.

CREDENTIALS:
Current STNA certification in State hired.
Current physical exam and/or free from communicable disease as required by
state law.
Availability of automobile during working hours with a valid driver’s license and automobile insurance (where appropriate)
