Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team serving U-City Forest Manor in St. Louis, Creve Coeur Manor in St. Louis, and The Pillars of North County in Florissant, Missouri.

This full-time Physical Therapist opportunity offers the chance to provide exceptional physical therapy services across multiple skilled nursing facilities while working with a collaborative rehabilitation team focused on improving patient outcomes.

As a Physical Therapist (PT), you will treat adult and geriatric patients while helping improve mobility, strength, balance, endurance, and functional independence. You’ll collaborate with Occupational Therapists, Speech-Language Pathologists, therapy assistants, nursing staff, and facility teams to provide comprehensive, patient-centered care.

Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety techniques.
• Deliver compassionate, evidence-based care focused on positive patient outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Missouri Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Ability to provide coverage across assigned facilities as needed.
