Continuum Therapy Partners is seeking a compassionate and dedicated Occupational Therapist (OT) to join our rehabilitation team at Soldiers Grove Health Services in Soldiers Grove, Wisconsin.

This flexible opportunity offers either a part-time schedule or consistent PRN hours for a licensed Occupational Therapist who wants to provide meaningful patient care in a supportive, collaborative skilled nursing facility.

As an Occupational Therapist (OT), you will work with adult and geriatric patients to improve independence, safety, mobility, and quality of life through individualized occupational therapy interventions.

Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in a skilled nursing facility setting.
• Address activities of daily living (ADLs), functional mobility, coordination, strength, cognition, and safety awareness.
• Help patients improve independence and participate in meaningful daily activities.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies.
• Provide compassionate, patient-centered occupational therapy services.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist (OT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Passion for providing high-quality, patient-centered care.
