Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team providing coverage at Taylorville Care Center in Taylorville, Illinois, and Montgomery Nursing and Rehab Center in Hillsboro, Illinois.

This full-time Physical Therapist opportunity offers the chance to provide exceptional physical therapy services in supportive, collaborative skilled nursing facilities where your clinical expertise makes a meaningful impact every day.

As a Physical Therapist (PT), you will work with adult and geriatric patients to improve mobility, strength, balance, endurance, and functional independence while collaborating with an interdisciplinary rehabilitation team committed to delivering high-quality patient care.

Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence.
• Monitor patient progress and adjust treatment plans based on patient needs.
• Collaborate with physicians, nursing staff, caregivers, and interdisciplinary team members.
• Complete accurate and timely documentation according to company, state, and federal guidelines.
• Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety techniques.
• Provide compassionate, evidence-based care focused on positive patient outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Illinois Physical Therapist (PT) license required.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.
• Strong communication, organization, and teamwork skills.
• Ability to provide coverage at assigned facilities.
