This position is for a Full Time Social Services Designee at our Skilled Nursing Facility in the Findlay, OH area. We have an excellent clinical team in place now and the person we hire for this position will enhance our staff even further.

The primary purpose of this position is to provide Social Service needs to the residents in accordance with current applicable federal, state and local standards, guidelines and regulations.

Supports residents through their transition to skilled nursing placement; navigating insurance coverage; rehabilitation stay support and services; good communication skills; maintains confidentiality; assists families with questions; completes MDS; progress notes; care plans; attends various meetings; oversees resident discharge planning back to the community.

REQUIRED QUALIFICATIONS

Bachelor’s degree required. Prefer gerontology or social work.
