Full-Time Physical Therapist Assistant Opportunity

Continuum Therapy Partners is hiring a Licensed Physical Therapist Assistant (PTA) for a full-time position at Sunrise Health Services in Milwaukee, Wisconsin. This opportunity allows you to work with adult and geriatric patients in a skilled nursing and rehabilitation setting while partnering with an experienced interdisciplinary therapy team dedicated to improving patient outcomes.

Physical Therapist Assistant (PTA) Responsibilities

• Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist.
• Assist patients with gait training, therapeutic exercise, balance activities, strengthening, transfers, and functional mobility.
• Monitor patient progress and communicate changes to the supervising Physical Therapist.
• Complete accurate and timely electronic documentation.
• Collaborate with physicians, nurses, Occupational Therapists, Speech-Language Pathologists, and the interdisciplinary rehabilitation team.
• Educate patients and caregivers on mobility techniques, safety, and home exercise programs.
• Maintain compliance with Medicare, state, federal, and company guidelines.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Wisconsin Physical Therapist Assistant (PTA) license required.
• Skilled nursing, long-term care, rehabilitation, post-acute care, or outpatient experience is preferred but not required.
• New graduate PTAs are encouraged to apply.
• Strong communication, teamwork, and organizational skills.
