Continuum Therapy Partners is hiring a licensed Physical Therapist (PT) to provide rehabilitation services across three skilled nursing facilities in the St. Louis and Florissant, Missouri area. This full-time position offers the opportunity to work with adult and geriatric patients while collaborating with an experienced interdisciplinary rehabilitation team dedicated to restoring mobility, independence, and quality of life.

A $5,000 Sign-On Bonus is available for this full-time opportunity.

What You’ll Do

As a Physical Therapist (PT), you will evaluate patients, develop individualized treatment plans, and provide evidence-based physical therapy services designed to maximize function and improve patient outcomes.

Primary responsibilities include:

• Complete comprehensive physical therapy evaluations and reassessments.
• Develop and implement individualized treatment plans.
• Provide therapeutic exercise, gait training, balance training, transfer training, strengthening, mobility training, and fall prevention interventions.
• Help patients improve functional mobility, endurance, safety, and independence.
• Collaborate with Occupational Therapists, Certified Occupational Therapy Assistants, Speech-Language Pathologists, Physical Therapist Assistants, nursing staff, physicians, and caregivers.
• Maintain accurate electronic medical documentation in accordance with Medicare, state, federal, and company requirements.
• Educate patients and families on therapeutic exercises, home safety, and mobility techniques.
• Participate as an active member of the interdisciplinary rehabilitation team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Missouri Physical Therapist (PT) license required.
• Skilled nursing, long-term care, rehabilitation, outpatient, or acute care experience is welcomed but not required.
• New graduate Physical Therapists are encouraged to apply.
• Excellent communication, organization, and clinical decision-making skills.
• Ability to travel between assigned facilities in the St. Louis and Florissant area.
