Continuum Therapy Partners is seeking a Physical Therapist / PT for a travel therapy assignment supporting Heritage Manor Health Center and Guardian Angels Health & Rehab in the Chisholm / Hibbing, Minnesota area.

This is a full-time travel opportunity within the Continuum Therapy Partners network. Continuum is the therapy provider supporting these facilities directly, which gives travelers clearer communication, better insight into the assignment, and direct support from a team that understands the clinical environment.

Estimated weekly gross pay is based on assignment details available at the time of posting. Actual weekly pay may vary based on hours worked, stipend eligibility, benefit elections, tax withholdings, and approved reimbursements. What You’ll Do

&#8226 Provide skilled Physical Therapy services in accordance with clinical standards, physician orders, plan of care, and facility expectations.
&#8226 Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life.
&#8226 Develop and update individualized treatment plans based on patient needs, goals, and progress.
&#8226 Collaborate with rehab leadership, nursing, facility teams, families, and care partners.
&#8226 Support discharge planning, patient education, and functional outcomes.
&#8226 Step into the assignment with professionalism, flexibility, and a patient-centered approach.

What We’re Looking For

&#8226 Active or eligible Minnesota Physical Therapist / PT license.
&#8226 Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred.
&#8226 Travel experience preferred, but not required for the right clinician.
&#8226 Net Health experience is a plus.
&#8226 Strong documentation, communication, and team collaboration skills.
&#8226 Ability to start quickly and complete required onboarding before assignment start.
