EnduraCare seeks a compassionate, caring and dedicated contributor to our TEAM.
One with a strong healthcare background specific to post-acute services. One
who understands the needs of each patient in our care. This key role will
direct, oversee and coordinate the development and completion of our residents''
assessments (MDS) in accordance with the current federal and state rules,
regulations and guidelines which govern resident assessment. This team
member will audit resident charts, care plans and charting to ensure
documentation supports the care provided and reimbursement level. We seek
someone who understands and is able to maintain complete confidentiality.
Minimum Job Requirements:
Graduation from an accredited school of professional nursing -
BSN or MSN.
Current licensure as a Licensed Practical Nurse, (where allowed
by state regulations), Registered Nurse (BSN or MSN) by the Board of Nurse
Examiners in respective state.
Minimum of 2 years prior inpatient medical-surgical experience
or skilled nursing experience.
Experience with completion and electronic submission of the MDS
Experience as a case manager or discharge planner preferred.
Knowledge and experience in federal and state regulations and
legislation related to provision of care in transitional care/skilled nursing
Effective verbal and written communication skills.
Organization, time management, and ability to work with others.
Demonstrated proficiency with computers, software and a variety
of office equipment. Navigation of electronic medical record.
Must exercise sound judgment in handling confidential
